Services and Supports to Discuss
Supportive Home Care
Assistance with daily routines and activities in the home or community, based on individual needs, approved service plans, and program requirements.
Respite and Caregiver Support
Short-term support intended to give eligible family caregivers or other unpaid caregivers relief, when available and authorized.
Chore and Household Support
Help with qualifying household tasks that support a safe and functional living environment, subject to program rules and availability.
Individualized Community Support
Other program-specific supports may be discussed with our team and the individual's care manager or service coordinator. Availability is not guaranteed.
Who May Be Eligible?
Eligibility depends on the applicable Wisconsin program, service authorization, individual needs, and payer arrangements. We can discuss referrals from individuals, families, guardians, care managers, and community partners. We do not guarantee enrollment or coverage through a particular Medicaid waiver or managed care organization.
